Evening, hope your well, go onto the gov web site and look for PIP select the fill out some information and they will send a form out, not quick and not easy to do/get I believe, I applied in April still yet to hear a response
I think you will find pip is for under 65 year olds and attendance allowance for over 65 (or current pension age)
