My son had a stroke 3 1/2 years ago. It has effected his right side. Will his immune system still be low after 3 1/2 years

I'm no medical expert but as far as I understand it and have experienced myself (It's almost ten years since my stroke), a brain injury does not affect our immune system. I am relatively well recovered and active, however.
If your question is related to worries about the Corona virus, in these difficult times, the best protection is social distancing, I suggest.
